Since then, our organization has evolved with a new name, as well as from offering services for adults with physical disabilities to serving those with a brain injury through eight facilities in the Region of Waterloo and County of Wellington. We are dedicated to the mission of maximizing our client's ability to live independently by providing support services for adults with physical disabilities or brain injuries.</span></p><p><span style="font-size:12pt;"><strong>A DAY IN THE LIFE OF A LIFE SKILLS FACILITATOR</strong></span></p><p><span style="font-size:12pt;">As a <strong>Life Skills Facilitator</strong>, you rely on your strong previous <strong>experience</strong> to learn the ins and outs of all your essential daily duties. This position is based out of our day program site, which provides services to adults with acquired brain injuries. Typical duties at the day program site include, but are not limited to: implementation of behaviour management techniques to ensure life skills programs are delivered effectively, teaching and retraining lost or impacted skills based on the analysis of client requirements (using very specific teaching and monitoring models), hands-on provision of attendant care and life skills programs, and you will work collaboratively with the Traverse Independence team, the client, his/her caregivers, family members, and other service providers to ensure the highest quality of services are delivered. You will <strong>communicate</strong> effectively with our clients and their families to ensure that they have the best quality of care. In addition, you will leverage your computer skills to keep up with charting and updating all records. This job requires you to have a <strong>flexible schedule</strong> and reliable transportation.</span></p><p><span style="font-size:12pt;"><strong>QUALIFICATIONS</strong></span></p><ul><li><span style="font-size:12pt;">Possess a Community College Diploma in Social Services, Health Care or related discipline</span></li><li><span style="font-size:12pt;">3-years experience providing personal care and working with complex clientele (experience supporting clients with disabilities is an asset)</span></li><li><span style="font-size:12pt;">Experience in behaviour management skills</span></li><li><span style="font-size:12pt;">Excellent organizational skills, interpersonal skills and communication skills (both written and oral)</span></li><li><span style="font-size:12pt;">Proven ability to work as a member of a multi-disciplinary team</span></li><li><span style="font-size:12pt;">Demonstrated computer proficiency and capacity to adapt to new software/systems</span></li><li><span style="font-size:12pt;">Existing knowledge of community resources is an asset</span></li><li><span style="font-size:12pt;">Valid Level C CPR/Standard First Aid Certification</span></li><li><span style="font-size:12pt;">Ability to pass a background check</span></li><li><span style="font-size:12pt;">Reliable vehicle</span></li><li><span style="font-size:12pt;">Valid class G driver's license in good standing</span></li><li><span style="font-size:12pt- Location Kitchener, ON
